Medical Services
Our services are provided by licensed medical professionals. By scheduling and receiving treatments, you acknowledge that:
Medical Consultation Required
All treatments require a proper medical consultation and assessment of your suitability for the procedure. Consultations are $200 and are credited in full toward your first treatment.
Informed Consent
You will be provided with detailed information about procedures, risks, benefits, and alternatives before any treatment.
Medical History
You must provide accurate and complete medical history and current health information.
Appointment Policy
Scheduling
- Eviaesthetics operates by appointment only; appointments must be scheduled in advance
- We require 24-hour notice for cancellations
- Same-day appointments are subject to availability
Preparation
- Follow all pre-treatment instructions provided
- Arrive on time for your scheduled appointment
- Bring valid, government-issued photo identification
Payment Terms
Payment is due at the time of service unless other arrangements have been made in advance.

- Pre-approved payment plans, where arranged in advance
Insurance
Eviaesthetics does not accept insurance. All services are provided on a private-pay basis. If you wish to seek reimbursement from an insurer or health savings account, we can provide documentation of services rendered upon request.
